It seems only to have gone pear shaped at the points where the train turned out onto the down refuge (not relief). The axle may have broken earlier but stayed pretty much in place until the lurch through the points, or it may have been the change of direction that finally caused the break. When it did break, everything still stayed upright, in line, and together, and merely wood-chipped some sleepers and ploughed ballast. I don't think the implications are as scary as you might think.
If this was in aviation, they'd probably ground and inspect a bunch of similar parts/aircraft. I wonder if there will be some panicked examination of coal wagons, or at least wheel sets from the same batch as the derailed wagon? Is this incident good enough to attract the ATSB or OTSI? It'll be interesting to read the report, if a major investigation occurs.

A similar thing happenned on an XPT relatively recently, the entire XPT fleet was grounded immediately the cause became apparent (many were stopped mid-run). Obviously the potential damages on a passenger train are different to that on a coal wagon, but potential damage and down time on infrastructure would get some people's attention.
I'd say there'll be a flurry of inspections, but that wagons will stay in service until their turn comes to be inspected.

As the accident didn't occur on the Defined Interstate Rail Network, I don't think ATSB investigate this one - OTSI will. Should be a pre-brief up in the next month or so.
