To add to that, I heard of cases where V/Line passengers were charged a default fare to Ballarat if they didn't touch off on trams in Melbourne.I cannot understand why that would be the case.

Under the pre-myki system, a V/Line fare from Ballarat included free travel in Melbourne (zones 1&2). Could be that some Ballarat folk think this is still the case, that all they need to do is touch on in Ballarat, then they are covered for metro travel as well. If they did not touch off at SC, and believed they did not need to touch on or off on a tram because tram travel was already included, then they would certainly be charged the default fare from Ballarat. If someone touch on at Ballarat and travels to any location and does not touch off at their location and then default fare will be applied and which is a zone 8 to 1 peak fare.
The only way this can happen if person travels from Ballarat to Southern Cross (touch on at Ballarat) and then does not touch off at Southern Cross and then hop on a tram service and touch on and then myki will show the user they have been charged a default fare from Ballarat to Melbourne ( zone 8 to zone 1 peak fare). That's what it's supposed to be, but as I said, based on what I heard from the conductor, that isn't the case. Don't ask me why as I don't know.
If the user arrive at southern Cross and touch off and hop on a tram service and touch on and then gets off the tram service without touch off and the default fare will be a zone 1

Not necessarily, because if they went through the fare gates they would have touched off after getting off the train. Are the gates ever closed at off peak time?
Are the gates ever closed at off peak time? They are all closed expect for one which I assume is left open for paper-ticket passengers.
I'm wondering because if they're closed passengers will be forced to touch off and pay an off peak fare.
If they're open they may just walk through and get charged a peak V/Line fare when they touch on a tram.
Guys at work who live in Riddles Creek and commute to the CBD are finding that even though they touch off at Riddles Creek at night, the next day their Myki balance is a lot lower than they expected. One of the chaps was told that his travel history showed that he travelled all the way to Bendigo. It's actually Riddells Creek
They're putting in for 1-2 refunds a month.
Also: why do Myki's expire?
Flocks of tourists with questions about their myki cards have put the brakes on a proposed $800,000 visitor information centre at Southern Cross Station.
A new tourist desk near the SkyBus terminal was trialled by Melbourne City Council between September and November this year. But instead of touting the city's top attractions, staff and volunteers found themselves explaining the intricacies of Melbourne's bus, tram and train ticketing system.
The helpers handled an average of 50 myki-related inquiries a day, with more than 60 per cent of visitor questions about public transport in general. Only a quarter of queries at the Western Gateway Visitor Service involved a service offered by the City of Melbourne and just a handful wanted to know about Docklands.
The council is now set to abandon an idea to build a council-run visitor centre at the station because of the volume of transport queries.
They physically wear out. Easier to just have them expire on a set date (where the user will actually know when to get it replaced by), than randomly physically break down due to an indeterminate cause later on.Mine didn't wear out after just short of four years (Feb 2010 to Oct 2013), I had to replace mine as it said I had 3 days to do so. Myki cards expire four years after they are issued.

Myki cards expire four years after they are issued. That's what I just said. They're intentionally set the card to expire earlier than it's expected to wear out so there's a regular timetable for getting them cycled through.
Re Fare increases - One that seems to have slipped under the radar as from 1.1.2014 the weekend capped rate has risen from $3.50 to $6.00 aleap of roughly 80 per cent. (also includes Public Holidays) The Sebiors cap has only gone from $3.80 to $3.90......................interseting................Doesn't seem to make the cap very useful, I rarely exceed spending $6 in the suburban area as it is.
