If he can't get COVID-19 under control in the US, the viability of holding the presidential election in November could be under threat.

No election means that the job of selecting the next president passes to the House of Representatives, which would probably result in the selection of a moderate from the House majority party.

If America gets to a million infections per day, then 250,000 deaths will be a gross underestimate. This virus is currently running at about 4% mortality, a million infections per day gives us an expectation of 40,000 deaths from just that day’s infections alone.

And if the US gets to that sort of infection rate, the mortality percentage will rise as fewer patients will have access to the supportive care needed to give them a chance of a genuine recovery (as opposed to being falsely declared "recovered" at a time that just happens to coincide with their insurance plan running out).
