The National Library "Trove" program locates many newspaper articles about a runaway plus derailment accident on Saturday 19 November 1910 at Heathcote Junction a little after midnight. There was apparently a brake failure due to a leaking pipe.
The Westinghouse brake was disconnected and replaced by application of hand brakes on some of the wagons. Hand brake levers were located on only one side of the wagons, so not all were applied. https://trove.nla.gov.au/newspaper/article/10478870
The train passed the Distant and Home signals and ran into the deadend at the junction. The driver, James Ryan, 43, was killed when the loco tipped over on its side.

I recall references to the Kurkuruc Creek bridge collapsing beneath a Dd way back when. A Gavan-Duffy manuscript circa 1935 mentions the event, as he describes a trip to Bendigo from Wallan on an AEC. I think the crew died, but passengers survived. Thankfully such bridge collapse events were rare.
The drivers union puts out “Loco Lines”, which several issues back reported the HJ incident. Really terrible crew conditions, driver unfamiliar with the track, two nights away from home. Cannot remember which issue, but I believe it was within the last 5 years. Do your own Google

From the reports, the initial cause was the 'total' failure of the Westinghouse brake. This led to the second cause: the train was not failed; someone - the railways or the crew - decided to bring it home on the handbrakes. Which led to the third cause: mismanagement of the braking after leaving Leslie. The driver could scarcely be blamed for this - his experience in running trains using handbrakes only on steep grades was likely to be limited if it existed at all.
With regard to the Coroner's findings - it's noteworthy that the VR had a number of accidents causing death due to brake 'failures' in this period. Beaufort. Sunshine. Kilmore Junction.
