Reports are stating this was a new bridge and on a nee line leading to a flaw in the design or the construction which is a terrible situation.

Aren’t these bridges regularly checked and any maintenance required completed.

The line is not new, it is 9 years old and would have handled millions of tonnes of traffic since then. So not a design flaw.

I question how thorough the inspection/required repairs were since the earthquake.

The viaduct has apparently been having issues for some time. There's a picture from last October showing a beam banana-ing on one side. In 2017 the support columns were noted to've had concrete falling from the structure and that there was insufficient reinforcement structure, the fix for that was add additional reinforcement - steel, carbon fiber - and applying resin and more concrete. Horizontal beams were also found adrift from the columns, answer wield in some steel diagonals and repair fractured concrete.

Nobody went to jail or was even found negligent over the decision to use cheap, flammable cladding on Grenfell Tower in the UK.
